My Support My Choice is a joint project between the Health and Social Care Alliance Scotland (the ALLIANCE) and Self Directed Support Scotland (SDSS). The aim is to establish how Self-directed Support (SDS) is working in practice for supported individuals across Scotland. The project seeks the experiences of anyone who receives SDS or has been assessed in the last 12 months, regardless of the outcome of this process.
The purpose of this project is to explore people’s experience of SDS, so that we can promote existing good practice or future change that will enhance the experience.
If you would like to share your experience you can complete the following anonymous survey. Please allow approximately 20 minutes to do the survey, in some cases it may be much faster.
If you have any questions or would like support to fill out the survey you can call our freephone support line – 0800 1701 321
Support is available Monday-Friday 9.30am-4.30pm.
